First point of contact

Milena Pamyan, Intake Coordinator is the first point of contact for all referrals and admissions. She confirms eligibility, verifies benefits, and schedules the admission visit.

For physicians & hospitals

Referrals accepted 24 hours a day.

Call (818) 273-4884 or fax clinical documentation to (818) 273-9136. Referrals are triaged on receipt by Milena Pamyan, our intake coordinator, who confirms eligibility with our medical director and reports back to your office the same day.

Attending physicians are encouraged to remain the patient’s primary physician of record and may continue to bill Part B. Our medical director, Dr. Iraj Zamanian, is available for peer-to-peer eligibility discussion.

What to send with a referral

  • Patient name, date of birth, and contact for the responsible family member
  • Terminal diagnosis and supporting clinical documentation
  • Recent history and physical, or hospital discharge summary
  • Current medication list
  • Insurance information and, if available, advance directive or POLST

Incomplete referrals are still welcome — we will gather what is missing rather than delay a patient in distress.
