A small team, deliberately kept small.
Rosewood was founded by a hospice nurse and a physician who believed caseloads should be small enough that a nurse knows every patient by name.
Mission
To honor the sacredness of every life to its final breath — caring for body, mind, and spirit with skill, humility, and love.
We believe each person is made for dignity, and that dignity does not diminish with a diagnosis. We believe dying is a part of living, not a medical failure. We believe pain should be treated seriously and quickly, that families are partners in care rather than visitors, and that honest information, offered gently and in plain language, is one of the kindest things a care team can give.
Rosewood is a faith-informed hospice. Our chaplaincy walks alongside people of every tradition and of none, and we never ask anyone to share our convictions in order to receive our care. What our faith asks of us is simple and demanding: to show up, to tell the truth, to relieve suffering where we can, and to make sure no one faces the end of life alone.
Rosewood began as a small office on Sherman Way with a handful of nurses and a single social worker. We have grown carefully since — adding a chaplain, a bereavement program, and a volunteer corps — while holding nurse caseloads deliberately below the industry average. We remain independent — licensed by the California Department of Public Health, accredited by The Joint Commission, and certified by Medicare and Medi-Cal.
The care team model
Six roles, one plan of care.
Every patient is served by an interdisciplinary team that meets every other week to review the plan together. You are not passed between departments; the same people stay with your family.
Nurse
Your case manager. Visits on a regular schedule, assesses symptoms, adjusts medications with the physician, and teaches families hands-on care.
Hospice aide
Bathing, grooming, mouth care, dressing, linens and gentle repositioning — the daily dignity work, done without hurry.
Social worker
Advance directives, benefits, placement, family meetings, counseling, and the practical logistics nobody prepared you for.
Chaplain
Spiritual and existential support on your terms, in your tradition or outside of any — including vigil presence and memorial planning.
Volunteer
Companionship, reading aloud, errands and respite sitting. Trained for thirty hours and matched by language, faith or shared interest.

Comfort first, always
Untreated pain is an emergency. We respond to symptom calls the same day and adjust plans within hours, not at the next scheduled visit.
Plain language
No euphemisms and no jargon. Families deserve to know what is happening, what to expect, and what choices remain open to them.
The family is the unit of care
We teach, we listen, and we keep supporting the household for thirteen months after a death. Caregivers are patients too.
Small caseloads
Our nurses carry fewer patients than the state average, on purpose. Continuity is not a luxury at the end of a life.
Licensure & accreditation
Who holds us accountable.
Anyone can say they provide good care. Oversight is what makes the claim checkable. Rosewood is inspected, surveyed, and certified by three separate bodies, and we would rather you knew that before you called than after.
Licensed by the California Department of Public Health
CDPH licenses and surveys every hospice agency operating in the state. Our license covers clinical practice, staffing, recordkeeping, and patient rights, and it is renewed on the state’s schedule of unannounced inspections.
Accredited by The Joint Commission
Joint Commission accreditation is voluntary and goes well beyond the legal minimum. Surveyors arrive unannounced, follow real patients through our records, and hold us to national standards for symptom management, safety, and family support.
Medicare and Medi-Cal certified
Certification means the hospice benefit covers your care in full — the visits, the medications related to the terminal illness, the equipment, and respite. There is no copay and no deductible for hospice under Medicare.
